背景 近年,透析治療の進歩による生存寿命の長期化と導入の高齢化により,高齢透析患者が年々増加している.透析治療の受容には,認知機能が影響することから,年齢が高くなるほど透析治療の受容は悪いことが指摘されている.しかし,高齢透析患者の多くは,透析治療を休むことや放棄することなく治療を継続しており,そこには透析の受容とは異なる療養生活における体験の意味づけがあると考えられる.目的 本研究は,高齢透析患者の療養生活における体験の意味づけについて明らかにすることを目的とした.方法 65歳以上の高齢透析患者8名を対象に,半構成的質問紙によりインタビューを行った.面接内容は,1)研究協力者の基本的属性,2)血液透析療法をしている生活について,3)透析日,非透析日の過ごし方について,4)療養上の工夫について,5)療養生活の中で見出している生きる意味,価値,目的,心の支え等で,研究協力者に自由に語ってもらった.面接内容の逐語録を作成し精読した.高齢透析患者の療養生活における体験に関連していると思われる文脈の1つの区切りごとにコードを作成し,分類整理し,下位カテゴリを抽出した.そして,下位カテゴリを比較検討し,中位カテゴリを抽出した.中位カテゴリを比較検討し,上位カテゴリを抽出し,上位カテゴリの関連を検討した.結果及び考察 【生ある者として生き抜く姿勢】,【円滑に過ごすための努力】,【暗澹としてやるせない】,【やるせなさとの折り合い】,【自己の存在価値の認識】,【療養生活における期待】の6個のカテゴリを抽出した.高齢透析患者は,療養生活において生への希求に基づいた【生ある者として生き抜く姿勢】を持ち,日常生活を自立して過ごせるように療養生活を【円滑に過ごすための努力】をしていた.その一方で,高齢透析患者は,療養生活に【暗澹としてやるせない】体験をしながらも【やるせなさと折り合う】ことで透析の療養生活を受容していた.高齢透析患者は,療養生活における人間関係を通して【自己の存在価値の認識】をする体験により【療養生活における期待】を見出し,療養生活における辛さからの解放や心の安定を感じる体験をしていた. 高齢透析患者が療養生活をより充実して過ごせるように,家族と連携して患者の認知機能や理解力に合わせた説明を行い,地域とも連携して支援する必要であると考える.結論 看護師には,高齢透析患者がより充実した療養生活を送ることができるように,療養生活における体験の意味づけを理解し,家族や地域との連携を深め,よりよい人間関係を構築し,適切な支援を得ることができるように支援することが重要であることが示唆された. |

Background It has been pointed out that the older the patients are,the more poorly they accept dialysis treatment because of a decline in cognitive function.I consider that they implicate their medical treatment livesas matters different from acceptance of dialysis. Objective The present study aimed to clarify the experiences of elderly patients with dialysis receiving medical treatmet. Methods I conducted a semi-structured interview with eight elderly patients with dialysis aged over 65 years.The topics in the interview included (1) basic attributes of the participants,(2) living with hem dialysis treatment,(3) days spent with and without dialysis,(4) devices used in medical treatment,and (5) the significance,value,purpose,and mental support received during medical treatment. Results/discussion Six categories,that is,"attitudes to survive as those with life," "efforts to spend life smoothly," "being depressed and disconsolate," "compromising melancholia," "recognition of the value of their own existence," and "expectations of living with medical treatment life," were extracted.When living with medical treatment,elderly patients wih dialysis were motivated to live and made efforts to live with medical treatment as best as they could.They wereinterpreting these as due to having higher expectations of living with medical treatment and using these experiences (recognizing the values of their own existence) to improve their human relationships while living with medical treatment. Conclusion It is important for nurses to empathize with elderly patients with dialysis receiving medical treatment and to deepen the cooperation with the patients' families and regions in supporting them. |
