Method The “problems and challenges” (kadai in Japanese) described by 34 nursing directors were categorized using content analysis. Results The problems and challenges that nursing directors at medium-sized hospitals face after taking office could be classified into 12 categories: “restructuring of the education system,” “new management perspective,” “dealing with matters related to both the hospital and outside organizations,” “latent organizational issues,” “a senseof pressure on responsibilities,” “a growing shortage of personnel,” “securing and utilization of human resources,” “appropriate personnel management,” “improvement of comfortable working environment,” “statement of department policy,” “morals and quality of nursing,” and “safety management.” Discussion The timing when the 12 categories of problems and challenges occurred were categorized into three: the stage immediately after appointment as nursing director, the stage when the nursing directors directly realized and grasped the situation of the organization, and the stage when they thought about an approach to organizational optimization.
